Athletic physicals will be held on Monday, May 24 at 1:00 p.m. in the Holy Cross Student Center for students in grades 7—11 who plan on participating in LHSAA sports. Parents must complete the first section of the form which includes student information, family medical history, athlete's orthopedic history, athlete medical history, parent waiver form which must be circled yes on needed approval, and signature of parent. All above listed sections on the physical form must be completed by the parent and brought to the physical with the student or the student will not be allowed to participate in the physical. The only portion that should not be filled out is the bottom section. This is for the doctors to complete. E-copies or digital copies of form will not be accepted at the physical on May 24. The Ochsner doctors present will need the completed physical copy of the form to sign off on.
The cost of the physical is $10.
